If you want the Everyday Math Reference textbook to be sent home to help you understand the math for your child’s homework and Math Boxes, please copy and sign this form with your child’s name and return it.  This textbook may remain at your home until you either change schools or until the end of this school year. If you do not return this textbook at the end of the year, you will have to pay $32.
